LIABILITY DISCLAIMER AND MEDICAL AUTHORIZATION:
I do hereby relieve the Assumption Parish, the Assumption Athletic Association board and it's members from any and all liabilities, including personal injury, arising out of or pertaining to participation of your child in athletic programs and activities of Assumption Parish in connection to this registration.
I further authorize the manager or coach of my child's team to secure any and all necessary medical aid, assistance, or treatment for my child during any practice or game if such medical aid, assistance, or treatment is required due to any injury to my child. I have read the above foregoing Liability Disclaimer and Medical Authorization and understand the contents thereof, and agree that the information and facts contained on my registration are true to the best of my knowledge and belief.
WORK DUTY : These terms apply only if you did not buyout of your work duty at registration.
When you perform your work duty assignment you must sign the work duty signoff sheet. If a work duty signoff sheet is not available, you must contact the work duty coordinator to confirm you fulfilled your work duty requirements. Failure to sign the sheet or contact the work duty coordinator may result in you being assessed a “No Show" buyout fee – see below.
With at least a 72 hours notice of your scheduled work duty you may contact the work duty coordinator and request a reschedule. If you cannot find a substitute and your work duty cannot be rescheduled, you must buyout of your duty at a cost of $100.00.
If you are within 72 hours of your scheduled duty, and you cannot work, it is your responsibility to find a substitute. Failure to find a substitute may result in you being assessed a "No Show" buyout fee of $150.00.
If you have a substitute you must contact the work duty coordinators to let them know who is working your shift.
Failure to perform your required work duty will result in you being assessed a “No Show" buyout fee of $150.00. A hold will be placed on your account and you will be unable to register for subsequent sports until the buyout is paid. It is your responsibility to contact the work duty coordinator to have the matter resolved.
You be notified via email or phone call if your work duty shift is cancelled. If your work duty shift is cancelled you will be placed in the work duty pool for the purpose of working another work duty shift later in the season.

REFUNDS:
All requests for refunds must be submitted in writing (email is acceptable) to the appropriate Lay Director, indicating the reason for the request.
- If a child can not be placed on an Assumption team, registration fees are fully refunded, the administration fee is waived, and your work duty requirement (if applicable) is also waived.
- Requests for refunds before team formations are complete will receive a full refund, less a $20 administration fee and any late fees paid at registration. Your work duty requirement (if applicable) will also be waived.
- After team formations are complete, no refunds will be granted, save in the case of season-ending injury or illness as approved by the Assumption Athletic Association executive board. Such refunds will come in the form of a pro-rated credit depending how far into the season the request is made. Your work duty requirement (if applicable) will remain in place and must be fulfilled.
CYC CARDS:
Every player, manager and coach participating in CYC sports must have a CYC identification card.
You will need a birth certificate, baptismal certificate, driver's license, or an old CYC card.
The cost of a card is $3.00.
Sign-up dates and times will be posted at www.cycstlscnty.net .
UNIFORM RETURN:
Uniform turn-in dates will be scheduled by your child's coach. It is your responsibility to ensure that your child's uniforms are returned. Failure to turn in a uniform will result in your child not being allowed to participate in subsequent sports.
CREDIT CARD AGREEMENT:
1) Sports signups are applicable to the upcoming CYC sports seasons, which may begin up to 2-4 months after your registration date.
2) Cancellations of upcoming sessions will be addressed on an individual basis, and should be addressed with the Assumption Athletic Association treasurer. Contact information is provided on the Assumption Athletic Association home page.
3) Any other disputes or questions regarding Assumption Athletic Association Sports signups will be addressed by the treasurer and/or board of the Assumption Athletic Association. Decisions made by the treasurer and/or board regarding said disputes/questions will be final.
